Sometimes a c220m3 with esxi wasn't showing the pci nic cards at all in the network adapter list.
I tried reseating the pci adapters but well but no go.
firmware was okay since the same was fine with many other servers. Lastly we ended up setting the MMIO above GGB to disabled in the BIOS to get it working. I still do not know how and why but a colleague suggested that this is what which works in this scenario and it is not the hardware or the firmware but just the setting. We also do not have to do this if everything is fine.
one more thing. make sure the CDN support too is disabled in the same place as the below screen.
